VHSL Sports Physical Form
In 2021, the VHSL released a new revision of the Sports physical form that is required in order to participate in athletics! In order to participate in athletics, students must have the form linked below completed and submitted to the WT Woodson Athletic Trainers.
- The current version will say "Revised January 2021" in the upper right corner.
- This is the ONLY version that will be accepted next year; if you see "Revised February 2017" in the top right corner of your form, you have the old version.
- In order for this form to valid for the 2022-23 SY, it must be signed and dated by the examining healthcare provider on or after May 1, 2022. - Please double check that all signature lines have a signature, there is at least one on each page!
